Insurers use different forms for traditional, investment-linked and protection-only plans. The wrong one can delay or void a request.
Aviva policies are now serviced by Singlife, AXA policies by HSBC Life, and NTUC Income is now Income Insurance.
Forms change. Download from the insurer's page each time, not from an old copy saved elsewhere.
Ask for your net surrender value first, and make sure you have looked at the alternatives.
